What blood pressure is too high?
Blood pressure is high when it stays at 130/80 or above. Most adults with high blood pressure aim to stay below 130/80 with treatment.
Normal
Below 120/80.
Elevated
120 to 129 on top, with the bottom number below 80. Lifestyle changes come first.
Stage 1
130 to 139 on top or 80 to 89 on the bottom. Treatment depends on your heart risk.
Stage 2
140/90 or higher. Most people need medicine as well as lifestyle changes.
A reading of 180/120 or higher with chest pain, shortness of breath, weakness, vision change or trouble speaking needs 911. Without symptoms, wait 5 minutes, recheck, and call us the same day if it is still that high.
How to check your blood pressure at home
Home readings help Dr. Munib adjust your medicine with fewer visits. A cuff that fits your upper arm gives the most reliable numbers.
- No caffeine, exercise or smoking for 30 minutes before.
- Sit quietly for 5 minutes with your back supported and feet flat.
- Rest your arm on a table at heart level, cuff on bare skin.
- Take 2 readings a minute apart, morning and evening, and bring the log or the cuff to your visit.
Prediabetes and diabetes: which numbers and checks matter
An A1c of 5.7 to 6.4 percent is prediabetes. 6.5 percent or higher on two tests is diabetes. Prediabetes is a chance to act: losing 5 to 7 percent of your body weight and getting about 150 minutes of activity a week lowers your chance of developing type 2 diabetes.
A1c
Every 3 months while your treatment is changing, and about twice a year once you are at goal. For many adults the goal is below 7 percent.
Every year
Kidney blood and urine tests, a foot check, cholesterol, and a dilated eye exam with an eye doctor.
Every visit
Blood pressure, weight, and a review of your medicines and any low blood sugars.
Brand-name GLP-1 medicines (Wegovy, Ozempic, Zepbound and Mounjaro) are an option for patients who qualify. The weight loss page explains them.
High cholesterol: when is medicine needed?
The decision is based on your overall risk of heart attack and stroke over the next 10 years, not on one cholesterol number. Dr. Munib estimates it from your age, blood pressure, smoking, diabetes and cholesterol, and adds family history and other factors.
- An LDL of 190 or higher usually means treatment, whatever the other numbers.
- Adults 40 to 75 with diabetes are usually offered a statin.
- After starting or changing a statin, cholesterol is rechecked in 4 to 12 weeks, then every 3 to 12 months.
- Tell us about muscle aches on a statin rather than stopping it on your own. A different dose or medicine often works.
Taking thyroid medicine the right way
Levothyroxine is absorbed most reliably on an empty stomach, which is a common reason levels are off.
- Take it with water 30 to 60 minutes before breakfast.
- Keep it 4 hours apart from calcium, iron and antacids.
- Your TSH is rechecked about 6 to 8 weeks after any dose change, then once or twice a year when stable.
- Tell us if you are pregnant or planning to be, because your dose usually needs to change.
Cost and insurance for ongoing care
Chronic condition visits and labs are billed to insurance as primary care. We are in-network with most PPO plans and accept HSA and FSA cards. Without insurance, visits are $190 for new patients and $150 for returning patients, and labs are paid to the lab at its self-pay price.

Frequently asked questions
Questions are welcome. Our team is happy to help before you book.
Call (949) 647-5234How often do I need a visit for high blood pressure?
About monthly after starting or changing a medicine, until your readings reach goal. After that, every 3 to 6 months is typical.
What A1c level means diabetes?
An A1c of 6.5 percent or higher on two tests means diabetes. 5.7 to 6.4 percent is prediabetes.
Can prediabetes be reversed?
For many people, yes. Losing 5 to 7 percent of body weight and staying active lowers the chance of it turning into type 2 diabetes.
Do I need to fast for a cholesterol test?
Usually not. Dr. Munib will tell you if your test needs fasting.
Can I take thyroid medicine with coffee?
Take it with water on an empty stomach, 30 to 60 minutes before coffee or breakfast. Coffee and food can lower how much you absorb.
Can a primary care doctor manage my diabetes?
Yes. Most type 2 diabetes is managed in primary care. Dr. Munib refers you to a specialist if your care needs it.
Is high blood pressure an emergency?
A reading of 180/120 or higher with chest pain, shortness of breath, weakness, vision change or trouble speaking is an emergency, so call 911. Without symptoms, recheck after 5 minutes and call us the same day.
Do you see patients from Yorba Linda and other Orange County cities?
Yes. Patients come from across Orange County. The office is at 114 Pacifica, Suite 280, Irvine.
